Scion Nonprofit Staffing has been engaged to conduct a search for a Clinical Case Manager for our client, an amazing Nonprofit organization focused on providing empowering resources and caring services to communities and individuals in need. This is an exciting Full-Time, Direct-Hire opportunity located onsite in San Francisco, CA!

This organization provides an excellent training program for registered AMFT, ASW, or APCC. Trainees can accrue up to 35 licensure hours per week! 


As the Clinical Case Manager, you will report to the Director of Clinical Services and utilize your social services knowledge to provide services to patients with mental health, substance abuse, and physical health conditions.  Your ability to work with a diverse group of people and commitment to the mission will help you succeed in this position!


RESPONSIBILITIES:
  • Provide case management, mental health services, diagnostic assessment, rehabilitation, and crisis intervention to clients and document services in AVATAR according to the organization’s and Medi-Cal guidelines
  • Conduct psychiatric assessment for medical necessity and clinical appropriateness for the program
  • Provide resources to clients on a broad range of problems in a manner that is supportive, constructive, and non-judgmental
  • Admit and orient new clients to the program
  • Develop and implement case management and money management plans and contracts
  • Coordinate care plans with other services being provided to the client
  • Provide advocacy assistance pursuing entitlements, clothing, health care, mental health services and other issues
  • Maintain entitlements and ensure proper completion of client applications and forms
  • Facilitate interaction among clients and involvement in the social program
  • Conduct support groups and activities oriented toward stabilized living
  • Maintain a supportive, independent living environment
  • Facilitate community building within the hotel and assist clients in utilizing hotel community resources
  • Intervene in crisis situations according to procedures
  • Assist clients with daily living skills
  • Meet monthly billing target
  • Develop and implement a plan for client placement upon discharge from program

QUALIFICATIONS:
  • 1-2 years of Case Manager experience
  • Registered AMFT, ASW, or APCC with the California Board of Behavioral Sciences or Board of Psychology
  • Master’s Degree in Clinical Psychology, Counseling, Social Work, or similar is required
  • Experience working with mental health and substance abuse patients
  • Knowledge of Medi-Cal guidelines is a plus!
  • Proficiency with AVATAR database is a plus!
  • Ability to work with people from diverse backgrounds
  • This position with our client requires employees to report to work at a physical location. COMPENSATION AND BENEFITS:
